Once you schedule your first appointment with us, you will receive a convenient Text Message containing a unique link to your New Patient Registration Forms. This link, when clicked, will direct you to the necessary forms. Please note that you will receive this message ahead of your initial appointment. We kindly request that you take a moment to complete these registration forms prior to your scheduled appointment.

Confirmation Text Message

You will receive a seperate text message with confirmation for the appointment- before every appointment you have. It is important for you to reply and confirm your appointment because if no response is received, your appointment could be cancelled from our end.

Valid GP Referral- for Medicare Rebates 

It's essential to note that valid referrals are crucial for claiming rebates, and these referrals are typically valid for a period of 12 months.
